1.  Terri is a 83-year-old widow who has been hospitalized and is being prepared for discharge. Terry was hospitalized after suffering a stroke on the right side. She has residual left-sided weakness, difficulty swallowing and communicating, and aspirates easily. The doctor has initiated aspiration precautions and her diet includes pureed foods and nectar thick liquids. While in the hospital, she receives PT, OT, ST, and assistance with her basic hygiene needs. Terri was independent at home, cooked her own meals, drove a car, and did her own laundry and housekeeping. Her closest relative is her 82-year-old brother who suffers from DM and is blind in the right eye. He lives 30 minutes away. How can Terri’s APRN direct and manage, or organize and coordinate, Terri’s discharge and who should be part of this process?

2.  What are decision support systems, and what role do they play in the business environment? Give an example of decision support systems used in health care. Discuss the challenges faced by EHR data mining. What is the potential of healthcare data mining? How can it benefit or improve patient outcomes?  Finally, explain why knowledge work and the clinical vocabulary contained in unified medical language systems are important for clinical reasoning and evidence-based practice.

Week 5 Written PaperChronically Ill Patient

Begin by identifying a patient from your clinicals who has a chronic health issue such as diabetes, COPD, or hypertension. Propose a healthcare coordination model that will benefit the specific patient and will also fit the clinic setting.

Your paper is to include the following information:

Description of the patient and his/her health issues, diagnoses, and current plan of care.

Description of the clinic setting, the healthcare providers (number and type), and the support staff.

Description of the patients/diagnoses typically seen in this clinic.

Description of the care coordination model being proposed. Please note, you may not use the OPT/CCCR model for this paper

Explain how this model will be effective for patients with similar diagnoses as discussed above.

Explain how this model will fit into the current clinic setting. What will need to change in the clinic setting to improve the fit?

The body of your paper should be a 3-4 pages long. This does NOT include the title page and reference page. Papers that are more than 4 pages long will not be accepted. Incorporate information from the course text plus a minimum of two (2) scholarly references published within the last five (5) years. Textbooks are not to be considered scholarly references.

Use current APA format to style your paper and to cite your sources. Your source(s) should be integrated into the paragraphs. Use internal citations pointing to evidence in the literature and supporting your ideas.
