Sarah is a 28-year Indigenous woman who is pregnant in her first trimesterwith her second child. She has been strongly advised to travel outside of her community to the city hospital to deliver her baby. She lives in a remote community north of Thunder Bay where there are no physicians or birthing hospitals. Today, Sarah is visiting her local community clinic for a prenatal checkup and assessment by the community health nurse. She brings her 11-month-old baby to the appointment.     This is her first prenatal check-up for this pregnancy. She admits she hasn’t attended regular prenatal appointments in the past because it is difficult to find transportation to the community clinic. It is also hard to find childcare because her husband works long days and nights as the sole income provider for the family.      On assessment, the nurse notes that Sarah is overweight and smells like cigarette smoke. Her medical history reveals that she had gestational diabetes with her first child. At first, she is hesitant to talk and maintains a downward gaze while speaking. She expresses concern about leaving to birth her child in the city as her husband would have to take time off work and she feels guilty for being away for so long. “When I delivered my last child in the city hospital, I went through depression so bad as I had to be there for so long by myself. I didn’t know anyone. I wish I was surrounded by my family and community.” She states she feels like a failure after this experience because she started eating poorly and smoking to deal with the depression.      The nurse begins teaching Sarah about prenatal health.
