MR is a 60-year-old man married for the last 26 years, two teenagechildren getting ready to go to college. He works at a call center after he was laid off from another job in 2018. He is a non-smoker, a keen cyclist, and drinks at least 20 beers a week. He has had left-sided testicle swelling. He looked up swelling in the testicle and he is worried that he may have testicle cancer. He does not have penile discharge or other sexual partners. He does admit that his marriage has run into difficulties due to reduced frequency of sexual intercourse with the stress of being laid off from his other job. He has had some urinary frequency in the last two weeks and his urine has been smelly but there has been no pain. He has a longer history of urinary frequency (years) but no hesitancy and a good stream. He attributes it to his prostate or perhaps the 4 cups of coffee he enjoys every morning but has no concerns about it and does not wish to have a treatment for it. He has no history of fever or chills. If he is told that he has an infection he wants to know how he got it, if it is an STD, it certainly was not due to him, and he wonders if his wife could have had an affair. He is also worried about cancer, and he is unable to afford to see a specialist because of paying for his kids’ college. Past medical history none, allergies none, he exercises four times a week. Temperature 37.5, pulse 90, BP 145/80, tender diffusely swollen left testicle with scrotal erythema, testicle sitting at the bottom of the scrotum, benign feeling slightly enlarged prostate. Urinalysis=no glucose, protein 2+, leukocytes 2+, nitrates 2+. Questions:1.
