These diseases are among the tope 10 conditions contributing to the mortality rate in the United States. The information would be collected through multiple resources. The resources would primarily be through websites such as CDC, CMS.org, and local health care facilities databases. Information in regard to how potential patients determine their hospital of choice of care would also need to be collected. Hospitals want to be able to market themselves in a good light, having a high readmission rate may deter some potential patients from attending.
