1. List any
triggers that seem to inflame or fuel this problem.

2. List any
behavioral risk factors which fuel this problem. (This includes risky or
questionable actions, etc.)

3. List any
environmental risk factors which fuel this problem. (This includes peers,
influences, population trends, toxins, etc.)

4. Identify your
greatest obstacle to change.

PART 3

(This section is based upon the information from the Heath
& Heath text. So, your answers for
each item below MUST include a corresponding citation from the text.)
