A teaching sheet for the patient regarding each of their medical diagnosis    Mr. Horton is an 89-year-old male admitted to the medical unit of the hospital with left leg cellulitis. Mr. Jones has a hx of Hypertension, Congestive Heart Failure, Depression, Arthritis, and Hypothyroidism. He is on a toileting regime and has to be assisted to the bathroom before and after meals to promote urinary and bowel continence. Mr. Horton also enjoys an afternoon nap. At 1200 the physician arrives to make rounds and review the patient’s blood work. Mr. Jones has low sodium.
