During the period of Steven’s incarceration, there was a case of active TB in the juvenile hall. The nurse is following up with Steven today for a positive tuberculin skin test and a negative chest x-ray. Additionally, the jail physician has prescribed Isoniazid daily for 6 months as a prophylactic measure.Steven was living on the streets before his incarceration. He left home at the age of 15. Steven’s father is a severe alcoholic who was physically and emotionally abusive to Steven. This man also physically abused Steven’s mother. Before leaving home, Steven attempted to convince his mother to leave but she refused. Steven has no siblings and is estranged from his extended family. He states that his ‘family’ now is his gang, a group of teens living on the streets that support themselves by panhandling and petty theft. Steven tells the nurse about his life on the streets. He informs the nurse that his ‘family’ travels around quite a bit and that they live in a renovated school bus. He adds that although they have “been hassled” by the police they are not involved in violent activities. He does admit that the group “likes to smoke weed a lot…but that just makes us mellow.”The goal for Steven is that he has an understanding of the importance of TB prophylaxis and that he continues taking his medication after his release from juvenile hall. The nurse would like to ensure that he have an understanding of the signs and symptoms of TB and appropriate follow-up measures should a problem arise.s:1.
