A 55 year-old male is evaluated for a 2-day history of fever, headacheand confusion. Medical history is significant for type 2 diabetes treated with metformin and prior renal transplant and patient is on Neoral (immunosuppression agent). Past surgeries included a splenectomy after laceration during blunt trauma. The patient has no known drug allergies.On exam, temperature is 102.6 degrees F, BP is 100/60 mmHg, pulse rate is 118 beats per minute and respiratory rate is 24 breaths per minute. He is confused but responds to vigorous stimulation. There are no rashes. Neurologic examination findings are non-focal. Lab studies show a leukocyte count of 22,000/ microL with 40% bands. A CT scan of the head is normal.
